Telehealth Physical Therapist Assistant – New York, NY (Remote)FOX Rehabilitation is growing in New York, and we're looking for passionate, licensed Physical Therapist Assistants who want to deliver exceptional one-on-one care to older adults via Telehealth. As a Virtual Care Specialist, you'll help patients improve mobility, independence, and quality of life through FOX's signature Geriatric House Calls™ model. Virtual Care Specialists primarily deliver care virtually obtaining truly meaningful patient outcomes while building strong, lasting patient you're searching for a high-impact PTA role that allows you to positively impact the lives of the patients we serve, FOX is the right fit for you!
Responsibilities:
  • Provide one-on-one physical therapy to older adults via telehealth, supplementing the virtual caseload with in-person house calls, SLC visits, and/or other patient care tasks as needed at the direction of the leadership team
  • Implement individualized plans of care developed by the supervising Physical Therapist and deliver evidence-based treatment
  • Deliver therapy sessions that support patient wellness, longevity, mobility, and functional independence
In coordination with the Supervising PT, complete and submit clinical and administrative documentation accurately and within required timeframes in accordance with the Clinical Documentation SOP, intake and tracking procedures, payer requirements, and audit standards Collaborate with an interdisciplinary team of PTs, OTs, SLPs, and Fitness Specialists Educate patients, caregivers, and senior living staff to promote safety, function, and aging in place Why Join FOX Rehabilitation? Qualifications:
  • Valid Physical Therapist Assistant license in the state(s) of practice, or eligibility to obtain
  • Degree from an accredited Physical Therapist Assistant program
  • Telehealth Experience
  • Proficient with Computer Skills
  • Proficient in Microsoft Office including Excel, PowerPoint and Word
  • Current CPR certification
  • Integrity, compassion, and enthusiasm
Physical Requirements: FOX clinicians must be able to meet the physical demands of in-home and community-based care, including:
  • Exerting medium physical effort, including lifting 25–50 lbs occasionally, 10–25 lbs frequently, and up to 10 lbs constantly
  • Performing CPRProviding moderate to maximum assistance with patient transfers and bed mobility
  • Repositioning patients, standing/walking for extended periods, climbing stairs, and performing therapeutic activities
  • Driving to patient homes and senior living communities as part of the Geriatric House Calls™ model
  • Wearing PPE when required
