Geriatrics NP | SNF Clinical Lead

Vitability-Health seeks an exempt Nurse Practitioner to provide direct patient care in skilled nursing facilities, leading and coordinating the SNF care team. The NP will manage care for both managed care and fee-for-service patients across SNF levels and meet patient needs with evidence-based geriatric medicine. The role requires active licensure as RN and NP in the state, plus preferred geriatrics certification and SNF experience.
