Collaborate with the care team to ensure a seamless member experience and accurately document coaching interactions
Required qualifications:
NBHWC Board Certification and professional Health Coach training from an accredited program
2+ years of experience in health coaching with a focus on behavior change or chronic disease management
Experience managing large caseloads effectively
Ability to deliver telehealth services in a private, professional space with reliable internet connectivity
Current residency in specified states within the United States
