Responsibilities: Performing thorough patient evaluations, ordering necessary diagnostic procedures, and developing tailored care plans Diagnosing and addressing acute, episodic, or chronic health issues, collaborating with physicians or independently based on your qualifications Informing patients and their families about prevention, wellness practices, and disease management Prescribing medications and therapies consistent with your licensure Engaging in patient follow-up and chronic care initiatives Working cooperatively with interdisciplinary teams to deliver integrated, comprehensive care Accurately documenting within electronic medical records (EMR) Education Criteria Master of Science in Nursing (MSN) or Doctor of Nursing Practice (DNP) Completion of an accredited Nurse Practitioner program
