Home Health Licensed Practical Nurse (LPN)

Aveanna Healthcare seeks an In Home Healthcare LPN to provide low-acuity skilled nursing care to pediatric and adult patients in their homes on weekdays and weekends. The LPN will follow physician-ordered care plans, administer medications and treatments, and monitor patient conditions while documenting care accurately. Responsibilities include patient and family education, supporting daily living activities, and maintaining safety and infection control standards. Provide compassionate in-home nursing care to low-acuity adult and pediatric patients under physician orders Administer medications, treatments, and wound care per care plan and document accurately in EMR Monitor patient vital signs, condition changes, and promptly report concerns to supervising RN or physician Educate patients and families on disease management, medications, and home care techniques Collaborate with interdisciplinary care team to update and follow individualized care plans Maintain infection control, safety, and regulatory compliance in the home setting Support patient mobility, personal care, and comfort within scope of LPN practice Participate in ongoing training, skills competencies, and quality improvement initiatives Home health nursing Medication administration Wound care Vital signs monitoring Electronic medical record documentation Patient and family education Care plan implementation Infection control Time management Collaboration with interdisciplinary teams Medical Insurance Dental Insurance Vision Insurance Life Insurance 401k Active LPN license in state of practice BLS/CPR certification Home health experience preferred Pediatric experience preferred Valid driver’s license and reliable transportation
