Your Responsibilities Include: Conducting thorough patient evaluations, requesting diagnostic procedures, and creating tailored care plans Diagnosing and managing acute, episodic, or chronic conditions individually or in collaboration with attending physicians (depending on your credentials) Providing education to patients and their families regarding prevention, wellness, and effective disease management Prescribing appropriate medications and therapies as per your scope of practice Engaging in ongoing patient follow-ups and chronic condition management Collaborating with multidisciplinary teams to guarantee seamless and comprehensive care Meticulously documenting within electronic medical records (EMR) Educational Background Master of Science in Nursing (MSN) or Doctor of Nursing Practice (DNP) degree Graduation from an accredited Nurse Practitioner program
