Healthcare Case Management Social Worker

Kaiser Permanente is seeking an LMSW Case Manager Social Work professional to join our Healthcare & Medical Services team. In this role, you will provide comprehensive case management for diverse patient populations, coordinating medical, behavioral health, and community resources to support optimal outcomes. Responsibilities include psychosocial assessments, individualized care planning, discharge planning, patient advocacy, crisis intervention, and collaboration with interdisciplinary teams. You will document services in the EMR, ensure regulatory and ethical compliance, and support quality improvement initiatives within a collaborative, patient-centered environment. Conduct comprehensive psychosocial assessments for assigned patients Develop and implement individualized care and discharge plans Coordinate medical, behavioral health, and community resources Provide crisis intervention and short-term counseling Advocate for patients and families to ensure access to services Collaborate with physicians, nurses, and interdisciplinary team members Document all interventions and plans in the EMR accurately and timely Ensure compliance with regulatory, ethical, and organizational standards Educate patients and families on treatment plans and community resources Participate in quality improvement and care coordination initiatives Clinical social work Case management Psychosocial assessment Care coordination Discharge planning Crisis intervention Motivational interviewing Electronic medical records (EMR) Knowledge of community resources Interdisciplinary collaboration Medical Insurance Dental Insurance Vision Insurance Life Insurance 401k Current LMSW license BLS/CPR certification preferred Experience with EMR systems Knowledge of Medicare/Medicaid regulations
