Description:
Conduct evaluations for patients in inpatient and outpatient settings and develop personalized care plans
Implement physical therapy interventions with a focus on enhancing mobility, strength, balance, and overall function
Work closely with an interdisciplinary team of nurses, doctors, case managers, and rehabilitation professionals to maximize patient outcomes
Educate individuals and their families about therapeutic goals, planning for discharge, and at-home exercises
Maintain accurate documentation of evaluations, treatments, and patient progress in compliance with institutional and regulatory standards
Engage in collaborative care team meetings and interdisciplinary roundtables to ensure seamless patient care
Education Doctorate in Physical Therapy (DPT) or equivalent from an accredited institution
Certifications:
Valid state licensure as a Physical Therapist
CPR certification is highly preferred
Additional specialized certifications (e.g., in geriatrics or neurology) will be advantageous but are not required
Skills Prior experience in a similar setting is strongly desired
Ability to manage complex medical and mobility-related issues
Excellent communication and teamwork capabilities
Proficient in digital health record systems and clinical documentation practices
