RN Case Manager - Home Health, Full-Time

Bridge Home Health and Hospice is seeking a registered nurse to serve as a Case Manager in Sonoma County, CA. This full-time role directs home care services, plans care with the patient and family, and coordinates with physicians to deliver nursing care in patients’ homes. The ideal candidate has a California RN license, CPR certification, and at least two years of licensed home health experience, with a bachelor’s degree preferred.
