Apply for this role (opens in a new tab)Now Hiring: Nurse Practitioner (NP) We are looking for a compassionate and experienced Nurse Practitioner to provide primary and ongoing care to residents in a skilled nursing facility. The NP will work closely with physicians, nurses, and the interdisciplinary team to ensure residents receive high-quality medical care.
Responsibilities:
- Conduct routine and acute patient assessments for nursing home residents
- Manage chronic conditions and develop treatment plans
- Respond to changes in residents' conditions and provide timely care
- Order and interpret diagnostic tests and labs
- Prescribe medications in accordance with New Jersey regulations
- Coordinate care with physicians, nursing staff, and families
- Ensure proper documentation in the EMRParticipate in care plan meetings and regulatory compliance
Requirements:
- Active New Jersey Nurse Practitioner license
- Board Certified Nurse Practitioner (FNP, AGNP, or GNP preferred)
- Experience in skilled nursing facility (SNF), long-term care, or geriatrics preferred
- Strong clinical decision-making and communication skills
- Ability to work collaboratively with nursing home staff
