PRN Pharmacist: Infusion Therapy & Antimicrobial Care

ContinuumRx is seeking a PRN Pharmacist to deliver safe, accurate infusion and specialty pharmacy services to patients transitioning from hospital to home. You will review orders, assess regimens, monitor pharmacokinetics, and collaborate with a wide clinical team to optimize therapy. You will ensure compliance with regulations, educate patients and caregivers, and participate in after-hours needs as part of the PRN team.
