In-Home Nurse Practitioner: Geriatric Health Evaluator

Matrix Medical Network is seeking a licensed Nurse Practitioner to perform in-home comprehensive health assessments. You will review medical history, medications, and living environment to identify care gaps and coordinate follow-up with PCPs and care teams. Travel to member homes or facilities is required, with flexible scheduling and full-time opportunities. Qualifications include an NP license, board certification, prescriptive authority, BLS/ACLS, and 1 year of NP experience preferred.
