Hospice Licensed Vocational Nurse (LVN) Job at Lorian Health in San Diego

Location: San Diego

Schedule: Friday (9:00am to 9:00pm) Saturday (9:00 am to 9:00 pm) and Sunday (9:00 am to 9:00 pm)

Job Summary: We’re looking for a skilled and compassionate LPN/LVN to provide in‑home nursing care under the supervision of a Registered Nurse (RN). You’ll support patients with daily care needs, monitor their condition, and help ensure safe, high‑quality home health services.

Responsibilities: Provide nursing care according to the RN‑developed Plan of Care. Monitor and document changes in patient condition; report to RN/physician as needed. Assist with activities of daily living to promote patient independence. Educate patients and caregivers within LPN/LVN scope of practice. Maintain accurate EMR documentation. Participate in case conferences and coordinate care with the home health team. Follow infection‑control, HIPAA, and company policies.

Requirements: Current LPN/LVN license in the state of practice. Graduate of an accredited practical/vocational nursing program. Minimum of one (1) year of recent (within the last three (3) years) experience in a Home Health or Hospice Setting. Current CPR/BLS certification. Valid driver’s license, reliable insured vehicle. Strong communication, clinical judgment, and ability to work independently.

Physical Requirements: Ability to lift/transfer patients and perform physical caregiving tasks. Ability to pass required physical exams and annual PPD testing.
