Your Responsibilities: Performing holistic patient evaluations, ordering relevant diagnostic tests, and developing personalized treatment plans Diagnosing and managing acute, episodic, or chronic health conditions alongside physicians or independently (as per your credentialing) Providing education to patients and their families on prevention strategies, wellness, and chronic disease management Prescribing medications and therapies in line with your professional scope Engaging in patient follow-up and chronic care management initiatives Working collaboratively with interdisciplinary teams to ensure comprehensive and coordinated patient care Maintaining accurate and detailed documentation within electronic medical records (EMR)

Qualifications: Master of Science in Nursing (MSN) or Doctor of Nursing Practice (DNP) Completion of an accredited Nurse Practitioner program
