Position: Social Worker
Location: Brooklyn, NY
Schedule: Full-Time | Monday–Friday, 9:00 AM – 5:00 PM
Salary: $60,000 – $65,000 annually

We are seeking an experienced and empathetic Social Workerto join our team in Brooklyn and provide essential support, guidance, and advocacy for residents and their families.
Key Responsibilities:
- Conduct resident and family interviews to develop comprehensive social histories and individualized care plans
- Partner with families to create programs that support residents’ emotional, social, and psychological well-being
- Assist with discharge planning, financial resources, and coordination of community support services
- Collaborate with an interdisciplinary team—including nursing, administration, and medical staff—to ensure holistic care
- Advocate for residents by addressing social and emotional needs and maintaining clear, effective communication
Qualifications:
- Master’s Degree in Social Work (MSW)preferred
- Bachelor’s degree required in Social Work, Sociology, Psychology, Special Education, Rehabilitation Counseling, or a related field
- Minimum ofone (1) year of supervised social work experiencein a healthcare setting, working directly with individuals
- Experience in a Skilled Nursing Facility (SNF)strongly preferred
- Strong interpersonal and communication skills with the ability to build trust and rapport
- Compassionate, organized, and committed to improving residents’ quality of life
