Primary Care Registered Nurse

Hartford Healthcare is seeking a compassionate Primary Care Nurse to join our Healthcare & Medical Services team in our integrated Connecticut network. In this outpatient role, you will deliver comprehensive primary nursing care, including assessments, triage, medication administration, chronic disease management, and patient education. You will collaborate closely with providers and an interdisciplinary team to coordinate care, support prevention and wellness, and ensure high-quality, patient-centered outcomes. Provide comprehensive primary nursing care for adult and geriatric patients in an outpatient setting Perform assessments, triage, and develop care plans in collaboration with primary care providers Administer medications, vaccines, and treatments while adhering to safety and quality standards Educate patients and families on disease management, prevention, and wellness strategies Document all care accurately in the EHR and support care coordination and follow-up Collaborate with physicians, advanced practitioners, and support staff to optimize patient outcomes Primary care assessment and triage Care planning and care coordination Electronic Health Record (EHR) documentation Medication administration and reconciliation Patient and family education Chronic disease management (e.g., diabetes, HTN) Wound care and basic procedures Vaccination and immunization administration Clinical quality and safety protocols Interdisciplinary team collaboration Medical Insurance Dental Insurance Vision Insurance Life Insurance 401k Active RN license in the state of practice BLS certification (AHA or equivalent) required ACLS preferred depending on practice setting Experience with EHR systems (e.g., Epic) preferred Certification in ambulatory care or primary care nursing a plus
