Participant Onboarding & Care Advocate

Habitat Health seeks a New Participant Advocate to welcome and onboard participants into the PACE program. You will build trusted relationships with participants and caregivers, coordinating with the Interdisciplinary Team to ensure a smooth transition into care and ongoing support. You will educate clients about services, coordinate appointments and transportation, and document interactions in the EMR. Travel within the service area is required as part of the role.
