Join NewYork-Presbyterian Hospital/Weill Cornell as an RN Care Management Coordinator, where you will provide intensive care management for pediatric patients with complex medical needs. Schedule: Full-Time, Monday-Friday 9:00am-5:00pm
What You'll Do:
Develop individualized care plans for pediatric patients.
Collaborate with an interdisciplinary team to provide comprehensive care.
Minimize and prevent hospital readmissions for patients with special needs.
Guide patients and caregivers through primary and specialty care.
Follow up with patients and families to ensure understanding of care plans.
Manage medication and home care needs for patients.
Utilize nursing expertise to address each patient’s unique needs.
Form strong bonds with patients, families, and multidisciplinary colleagues.
RequirementsEducation: Bachelor's degree in Nursing (BSN) required
Master's Degree in nursing or healthcare related field preferred
Experience:
5+ years of healthcare experience required
2 years of case management in an inpatient, HMO, insurance, or outpatient/ambulatory setting preferred
Licenses & Certifications:
Current NYS Registered Nurse (RN) licensure required
Bi-lingual – English/Spanish preferred
