Job Description:
The Home Health Physical Therapist will provide high-quality care in a home health setting, focusing on evaluations, treatment plans, and therapy interventions. This role requires experience with OASIS documentation and the ability to manage a productive caseload, averaging 4-6 visits per day within specified geographical areas. The role may involve travel within a 60-mile radius of the facility.
Responsibilities:
Conduct patient evaluations and develop treatment plans.
Implement therapy interventions and monitor patient progress.
Document care plans and patient encounters using Homecare Homebase.
Collaborate with other healthcare professionals in case conferences.
Ensure compliance with all regulatory and agency standards.
Required Experience / Certifications / Licensure:
Licensed Physical Therapist with prior experience in home health.
Experience with OASIS required.
Homecare Homebase documentation experience preferred; training may be available.
