Geriatric Physical Therapist Assistant

Renewal Rehab is seeking a compassionate Physical Therapist Assistant to support high‑quality rehabilitation services for seniors in skilled nursing and post‑acute care settings. Working under the direction of a Physical Therapist, you will implement treatment plans, lead therapeutic exercises, document patient progress, and collaborate with an interdisciplinary team. Implement physical therapy treatment plans under PT supervision Provide therapeutic exercises, gait training, and balance activities Assist with transfer training and functional mobility interventions Observe, measure, and document patient progress and responses to treatment Communicate effectively with supervising PT, nursing, and interdisciplinary team Educate patients and families on home exercise programs and safety Ensure accurate, timely, and compliant clinical documentation Adhere to all regulatory, ethical, and facility guidelines for senior care Contribute to discharge planning and recommendations for continued care Participate in team meetings, in-services, and quality improvement initiatives Therapeutic exercise implementation Gait training and balance intervention Functional mobility and transfer training Geriatric rehabilitation techniques Skilled nursing/post-acute care practices Patient assessment and progress monitoring (PTA level) Clinical documentation and EMR proficiency Patient and family education Infection control and safety protocols Interdisciplinary team collaboration Medical Insurance Dental Insurance Vision Insurance Life Insurance 401k Active Physical Therapist Assistant (PTA) license in the state of practice Graduation from an accredited Physical Therapist Assistant program BLS/CPR certification Experience or clinical rotations in geriatric or skilled nursing rehab preferred Familiarity with Medicare and post-acute documentation requirements preferred
