What You’ll Do: Conduct psychosocial assessments of clients and families Perform home visits to evaluate living environments Develop and implement individualized care plans Serve as the main point of contact for clients and families Coordinate community resources and support services Lead client and family meetings Collaborate with clinical and interdisciplinary teams Educate families on care plans and available resources Ensure accurate documentation and compliance
