Company Description: East to West Functional Medicine is a primary care practice dedicated to providing a functional, comprehensive, patient-centered care approach to individuals and families across the Treasure Valley. Role Description: This is a part-time or full-time, hybrid in clinic and telemedicine Nurse Practitioner role based in Meridian, ID. The Nurse Practitioner will provide comprehensive primary care, including health assessments, routine examinations, diagnosis, and management of acute and chronic conditions for patients across the lifespan. Daily responsibilities include obtaining medical histories, ordering and interpreting diagnostic tests, specialty functional lab panels, developing and adjusting treatment plans, prescribing treatment plans, supplements and medications in accordance with state regulations, and assisting patients with acheiving longevity/wellness. The role involves patient education on disease prevention, lifestyle modification, and integrative RX and medication management, as well as collaborative care with physicians, nurses, and other healthcare professionals to coordinate services and referrals. The Nurse Practitioner will document all encounters in the electronic health record, participate in quality improvement initiatives, and contribute to a supportive, inclusive, and culturally sensitive care environment.
Qualifications: Current Nurse Practitioner licensure (FNP or equivalent) in the state of Idaho, with active, unrestricted license and prescriptive authority, or eligibility to obtain prior to start. Graduate degree from an accredited Nurse Practitioner program and national board certification (e.g., AANP, ANCC).Demonstrated experience in family medicine, primary care, or a closely related outpatient clinical setting, AS WELL AS experience and training in functional medicine. Strong clinical assessment, diagnostic, and evidence-based treatment planning skills, including management of common acute and chronic conditions. Proficiency with electronic health records, clinical documentation, and basic health information technology tools. Excellent communication and interpersonal skills, with the ability to build trust, provide clear patient education, and collaborate effectively within a multidisciplinary team. Ability to work independently, manage time effectively, and prioritize patient needs in a fast-paced outpatient environment. Current Basic Life Support (BLS) certification; Advanced Cardiovascular Life Support (ACLS) certification preferred. Willingness to participate in quality improvement, practice development, and occasional community outreach activities.
