Licenses, Certifications, Requirements: Active NY RN License, Active New York Nurse Practitioner License, ACLS (American Heart Association), PALS (American Heart Association), DEA registration with New York address and schedules II, IIN, III, IIIN, IV, and V, Current NCCPA certification, Current Family Nurse Practitioner certification through AANP or ANCC. EMR: Meditech
