Your Responsibilities: Performing thorough patient evaluations, arranging diagnostic tests, and developing personalized care plans Diagnosing and managing acute, episodic, or chronic health conditions independently or in collaboration with physicians based on your scope of practice Providing education to patients and their families on health promotion, prevention, and managing ongoing conditions Prescribing necessary medications and therapeutic interventions within your professional guidelines Participating in chronic care management and patient follow-up Working alongside interdisciplinary teams to ensure seamless and integrated patient care Documenting accurately and comprehensively in electronic medical records (EMR)

Educational Qualifications: Master of Science in Nursing (MSN) or Doctor of Nursing Practice (DNP) Graduation from an accredited Nurse Practitioner program
