Registered Nurse (RN) - Case Management

Schedule: 8a-4:30p, Sun-Thurs Assignment Details: Guaranteed Hours: 72 Hours Biweekly Contract Weeks: 52

Requirements: Bachelor's Degree or equivalent; or Associate's degree in Human Services field preferred. Two to three years of related experience and/or training in the area of chemical dependency. Knowledge of relevant community resources for continuing care/aftercare placement. Certified Addiction Counselor (CAC) preferred. Must complete on-site CPI and CPR training/certification. Ability to relate sensitively to a substance abuse treatment population.

Responsibilities: Maintain individual case load, conducting intake evaluations, treatment planning, and safety planning. Complete documentation in a timely manner, as required by regulations. Coordinate with collaterals and referral sources as needed. Assist in retrieving patient medications from local pharmacies. Support staff in milieu management and crisis management. Engage in identifying and locating contraband and performing unit searches as needed. Assess patient's continuing care needs and arrange for transfer or admission to appropriate programming. Attend staff meetings and supervisory sessions as scheduled.
