Home Health Licensed Vocational Nurse (LVN)

Aveanna Healthcare is seeking an In Home Healthcare LVN for low-acuity patients on weekday nights. This role provides direct bedside nursing care in patients’ homes, including assessments, medication administration, basic wound care, and monitoring of stable chronic conditions. The LVN follows physician orders and individualized care plans, documents in the EMR, and communicates changes in status to the RN and clinical team. You’ll collaborate closely with families, support safe routines, and uphold Aveanna’s standards of compassion, integrity, and clinical excellence while working independently in the field. Provide direct low-acuity nursing care to patients in their homes on weekday night shifts Perform comprehensive assessments and monitor chronic, stable conditions Administer medications and basic wound care per physician orders and care plans Accurately document care and patient status in the electronic medical record Communicate changes in patient condition to supervising RN and clinical team Educate patients and families on disease management, safety, and home care routines Follow infection control, safety, and emergency protocols in the home setting Collaborate with interdisciplinary team to update and implement care plans Maintain professional boundaries, confidentiality, and regulatory compliance Travel between patient homes and represent Aveanna with compassion and integrity In-home patient assessment Tracheostomy care (if applicable) Ventilator monitoring (if applicable) Medication administration Wound care Care plan documentation Electronic health records (EHR) Vital signs monitoring Patient and family education Infection control Medical Insurance Dental Insurance Vision Insurance Life Insurance 401k Active LVN/LPN license in state of practice Current BLS/CPR certification Pediatric or home health experience preferred Valid driver’s license and reliable transportation Background check and health clearance
