LMSW Inpatient Case Manager

Kaiser Permanente is seeking an LMSW Case Manager to provide inpatient social work services within our technology-enabled care environment. In this role, you will conduct psychosocial assessments, coordinate discharge planning, and connect patients to community resources using our integrated digital platforms. You will collaborate with physicians, nurses, and care teams, document in electronic health records, and help optimize patient flow.
