Job Summary:
The Clinical Coordinator provides leadership and coordination of clinical pharmacy services across the acute-care setting, Working collaboratively with the Director and Assistant Director of Pharmacy, the Clinical Coordinator promotes safe, effective, evidence-based, and cost-conscious medication use for patients across all age groups, The position coordinates clinical pharmacy activities, supports quality assurance and performance improvement initiatives, and collaborates with physicians, nurses, pharmacy staff, and other healthcare professionals to advance medication-use practices and patient outcomes, The Clinical Coordinator coordinates clinical activities of pharmacy specialists and affiliated faculty practitioners to promote consistent clinical practice and alignment with departmental priorities, Coordinates the development, implementation, and assessment of pharmacist education, training, and clinical competencies, The Clinical Coordinator serves as a departmental clinical resource and subject matter expert; supports the education and development of pharmacists, residents, students, and other healthcare professionals; and provides leadership for regulatory compliance and medication-use initiatives, The position ensures clinical pharmacy services are provided in accordance with applicable organizational policies, accreditation standards, and federal and state laws and regulations,
Education, Years of Experience, Knowledge, Skills and Abilities RequiredGraduate of an ACPE accredited College of Pharmacy or FPGEC certification and internship hours
Completion of ASHP accredited PGY-1 Pharmacy Residency or relevant clinical pharmacy experience (3 years) in an acute-care hospital setting
Significant knowledge and expertise in advanced pharmacy practice, specialized medication therapy management, and specialty evidence-based medicine in area appropriate for clinical assignments
Demonstrated experience providing clinical pharmacy services in an acute-care hospital setting
Interprofessional Communication: Ability to lead multidisciplinary rounds and build consensus among physicians, nurses, and leadership
Teaching/Preceptorship: Skills to educate and mentor pharmacy staff, residents, and pharmacy students
Project Management: Ability to design, execute, and monitor institutional health programs from start to finish
Education, Years of Experience, Knowledge, Skills and Abilities PreferredDoctor of Pharmacy or other relevant advanced degree
Completion of PGY-2 Specialized Pharmacy Residency or relevant clinical pharmacy experience (5 years) in an acute-care hospital setting
Pharmacy leadership experience
Antimicrobial Stewardship (AMS) experience
Experience with medication-use financial stewardship, including drug utilization evaluation, cost-containment initiatives, and budgeting
Proficiency in data analytics and visualization, including the use of analytical tools to develop dashboards, evaluate clinical outcomes, and monitor performance improvement initiatives
Demonstrated experience developing, implementing, and evaluating evidence-based clinical guidelines, protocols, pathways, or medication use initiatives
Licenses and Certifications:

Essential Job Functions1, Provides integrated pharmacy services in a decentralized practice setting with providers, nurses, and other hospital personnel, Conducts regular, patient-centered chart reviews, clinical assessments, and bedside rounding,
2, Develop, implement, evaluate, and optimize evidence-based clinical guidelines, protocols, quality improvement initiatives, research activities, and policies in collaboration with multidisciplinary patient care teams,
3, Participate as a representative on committees, system collaboratives, or service line meetings as a liaison for the pharmacy department or facility,
4, Provides leadership and support for formulary management and medication-use evaluation, incorporating evidence-based practice, medication safety, health-system alignment, clinical outcomes, and financial stewardship,
5, Provides clinical pharmacy oversight of medications in infusion pump libraries, including medication parameters, concentrations, dosing limits, and associated medication-safety initiatives,
6, Develops and oversees clinical education programs and clinical competency instruction for all pharmacists, Supports the medication information education for nurses, providers, and respiratory therapists, Actively participates in governance, teaching, education, and monitoring of students and pharmacy residents,
7, Direct report oversight of clinical specialists, anticoagulation trained staff, ICU trained staff, and coordination with collegiate faculty clinical specialists,
8, Provides leadership in establishing, monitoring, and improving the provision of pharmaceutical care, Prepares agenda and attends meetings as appropriate including pharmacy and therapeutics committee, interdisciplinary rounds, infectious disease rounds, etc,
9, Collaborates with pharmacy leadership, operations, procurement, and clinical stakeholders to evaluate drug shortages, identify clinically appropriate alternatives, develop mitigation strategies, and communicate medication-use recommendations,
10, Adheres to RWJBH Organizational competencies and standards of behavior as well as departmental specific standards of practice,
11, Provides pharmacy leadership for the Antimicrobial Stewardship Program in collaboration with infectious diseases physicians, infection prevention, microbiology, nursing, and other multidisciplinary stakeholders, Supports development, implementation, monitoring, and evaluation of antimicrobial stewardship initiatives in accordance with applicable regulatory and accreditation requirements,
12, Develops, monitors, analyzes, and reports clinical pharmacy performance metrics, medication-use outcomes, quality indicators, and regulatory measures; identifies opportunities for improvement and leads implementation of corrective or performance-improvement initiatives,
13, Serves as a pharmacy clinical resource for medication safety, regulatory and accreditation readiness, and development of medication-use policies and procedures,
