LCSW LMHC LMSW Director of Transitional Care - Bethpage, NY

What You’ll Do: Provide administrative and clinical leadership of both program components: The Specialty Critical Time Intervention (CTI) Team and the Transitional Residential Setting (TRS). Ensure integration and fidelity of both models and promote clinical excellence. Guide the team in delivering trauma-informed, person-centered care in accordance with the New York State Office of Mental Health (OMH) and the Office for People with Developmental Disabilities (OPWDD) standards.

What You Bring: Master’s Degree in social work, psychology, counseling, or related field. LCSW, LMHC, LMSW, or similar licensure; CTI and 9.58 certifications (within 12 and 6 months, respectively); Complete I-CONECT training through OMH within 6 months of hiring. Minimum of 5 years of supervisory experience and a minimum of 7 years of experience working with people with I/DD and/or Mental Health Diagnoses, with at least 3 years being post-licensure experience. Strong leadership, communication, and organizational skills, with a heart for collaboration and excellence. Computer literacy and experience with Testing/Screening instruments.
