Responsibilities:
The Physical Therapist will provide skilled physical therapy services, including but not limited to: Physical Therapy Evaluations ReassessmentsSkilled Physical Therapy Treatment VisitsGait and Balance Training Functional Mobility and Transfer Training Therapeutic Exercise Programs Neuromuscular Re-education Fall Prevention and Home Safety Education Patient and Caregiver Education Development and Implementation of Individualized Treatment Plans Documentation and Care Coordination Compliance with Physician Orders, Regulatory Requirements, and Agency Policies The Occupational Therapist will provide skilled occupational therapy services including, but not limited to:
- Occupational Therapy Evaluations
- Reassessments
- Therapeutic Treatment Visits
- Documentation and Care Coordination
- Patient/Caregiver Education
- Development and Implementation of Treatment Plans
- Compliance with Physician Orders and Agency Policies Qualifications
Qualifications:
- Active Physical/Occupational Therapist - Licensed in applicable jurisdiction(s)
- Graduate of an accredited program
- Strong clinical assessment and treatment planning skills
- Excellent communication and interpersonal abilities
- Ability to work independently and collaboratively with interdisciplinary healthcare teams
- Home health experience preferred but not required
