What You'll Do:
Assess, plan and implement care strategies individualized by patient.
Identify and initiate referrals for social service programs.
Manage the care plan throughout the continuum of care.
Communicate required health-related information to stakeholders.
Advocate for patients and families to ensure their needs are represented.
Requirements:
EducationCurrent unrestricted Texas RN license required.
Experience 2+ years of experience in a community health or healthcare setting.
Licenses & Certifications:
Current RN License in Texas.
