Nurse Program Coordinator: Headache clinic and Neurology infusion center

Responsibilities: 1. Assesses patient/families physical, emotional, spiritual, psychosocial and learning needs; communicates pertinent information to the team and other health care providers; assess and recognize changes in patient condition and family dynamics.2. Provides care coordination and care planning for target population3. Provides nursing care to target population of patients and documents evaluation and care delivery in patient plan of care.4. Determines the education needs of nursing personnel, multi-disciplinary team members and others; implements patient and family education in collaboration with the multi-disciplinary team.5. Provides leadership to the nursing team and acts as a change agent to improve patient care.6. Serves as a resource to other nurses and healthcare providers with expertise in assigned practice area.7. Other duties as required.

Other information: Technical Expertise1. Experience in pediatrics is required2. Experience in communication, teaching skills, and problem-solving skills is required3. Experience working with all levels within an organization is preferred4. Experience in healthcare is required5. Proficiency in MS Office [Outlook, Excel, Word] or similar software is preferred.
