Permanency Case Planner - Child Welfare & Reunification

Heartshare is seeking a dedicated Case Planner to help children achieve permanency and safety in care. You will coordinate services, liaise with ACS and community resources, and partner with families and clinicians. You will work with medical, mental health, and education teams, document progress, attend court hearings, and support reunification efforts through collaborative case management. Applicants should be able to travel across New York City by public transit and occasionally evenings.
